INSURED
The listing on the COI for Insured must reflect the exact name of the contractor as found on the contract between Xanterra and the contractor. The only standing exception to this is when the contractor uses a labor service firm to provide employees to the contractor. In this case the Insured on the COI for workers compensation (ONLY) will be the labor company, with the contractor’s name included (ideally as Alternate Employer) shown in the Description of Operations section of the COI.
Assumed Names
For clarity sake, if an assumed name is used, both the assumed and legal name should be shown. For example, if the contractor is Bob Smith but has filed for an assumed business name in the name of Bob’s Painting Services, the Insured shown on the COI should read: Bob Smith dba Bob’s Painting Services. In cases of a sole proprietorship (not a sole owner LLC) the individual’s name alone may be acceptable but may result in additional information before evidence of insurance can be processed.
Assumed names are not just limited to individuals, any business entity can operate under an assumed name or trade name. For example: ABC Plumbing, Inc. dba Quick Plumbing Services. Both names should appear on the COI as the Insured.

ISSUER’S CONTACT INFORMATION
The ACORD certificate includes in the upper right-hand section space for including detailed contact information on who is responsible for the issuing of the certificate. This is very important information and can expedite any COI corrections when necessary. The Contact Name, Phone and Email Address needs to be completed.
INSURER(S) AFFORDING COVERAGE
Insurer’s Name
The stated AM Best rating in most contracts is a minimum of A- with financial size of VIII. In order to verify that information the COI needs to show the true insurer’s name. For example, if the insurer is State Farm Mutual Insurance Company, the COI must show that entire name. Just showing State Farm does not provide enough information to determine the AM Best rating.
In all cases, the insurance company’s name is shown, and not the broker’s or agent’s name in this section.
Multiple Insurers
The COI is designed to accommodate different insurance companies for different insurance coverages. For example, when Casco Indemnity Company provides the General Liability coverage and Ohio Casualty Insurance Company provides the Auto Liability coverage Casco Indemnity Company is typically shown as Insurer A and Ohio Casualty Insurance Company as Insurer B. Then in the far-left hand column of the COI “A” is indicated for the General Liability block and “B” is indicated for the Auto Liability block.
DESCRIPTION OF OPERATIONS
You should only include any statement in this text area that exactly reflects the policy’s stand. We are not asking for anything more, and acknowledge that any COI does not amend or alter policy language. That said, for clarity sake even when Additional Insured is indicated by an “X” in the Add’l Insd column one of the follow two statements is the correct way to comply with contractual requirements.
Option 1
This shows the precise wording as included in the contract. For example:
The National Park Service and Xanterra Parks & Resorts, Inc. (Xanterra), their respective owners, parent companies, affiliates, officers, directors, employees, agents, successors and assigns are included as Additional Insureds.
Option 2
As an alternative to Option 1 a statement concerning automatic additional insured status is acceptable. For example:
Additional Insured is included as required by written contract.
CERTIFICATE HOLDER
This section is only the Address section, so the contents does not need to be precise. What you do need to keep in mind though is that COIs will be accepted only when sent through email. DO NOT MAIL. Therefore, here’s an accepted example of how to address the Certificate Holder. Nothing more than this is needed:
